Anal fissure or fissure in ano is one of the most painful conditions affecting the anal region. It is characterized by a tear or crack in the lower end of the anal canal caused by excessive straining of the bowels. The disease is commonly seen in young people.

Causes of the Anal fissure.

The etiology of anal fissure can be described as:

  • Constipation – Due to the curvature of the sacrum and rectum, when the hard stool passes down, it causes a tear in the posterior side of the anal
  • Due to surgical procedures like haemorrhoidectomy (if too much skin is )
  • Repeated
  • Overuse of laxatives and ointments (thinning of anal canal’s skin).
  • Diarrhoea, increased sphincter tone, and
  • Other causes include ischemia, sexually transmitted diseases, Crohn’s disease, venereal disease, Ulcerative colitis, and Tuberculosis.

Types of Anal fissure.

An anal fissure is classified into:

1. Acute anal fissure:

It presents as a deep tear (paper cut type) in the lower anal skin with a spasm of the anal sphincter. The condition presents with severe pain and constipation and with little inflammatory edema.

2. Chronic anal fissure:

If an anal fissure lasts for more than 6 weeks, it can be categorized into chronic type. It appears with an inflamed and indurated margin with scar tissue as the base. The ulcer (canoe-shaped) at its inferior margin had a skin tag that is oedematous called a sentinel pile. Chronic anal fissure which is less painful than acute one can cause repeated infection, fibrosis, and abscess formation and leads to fistula in ano.

Clinical features:

  • Pain – which is tearing, burning, and sharp in nature? Pain starts during or after passing the bowels. Sometimes the pain may last for one or more hours after
  • Bleeding – The bleeding appears as bright streaks on the stools on the toilet
  • Constipation – pellet-like
  • There will be a slight discharge in chronic

It must be kept in mind that digital and protoscopic examinations are contraindicated in acute anal fissures.

Ayurvedic view of Anal fissures:

Anal fissures are described by the name Parikartika in Ayurveda. The disease is explained in Ayurvedic Samhitas as one of the complications of Panchakarma treatments like Vamana (vomiting therapy) and Virechana (purgation therapy). This is caused when a person who is weak and has low digestive fire consumes food items that are very strong, dry, and purgative in nature.

The disease appears to have clinical features such as:

  • Burning sensation and pricking pain in the anal canal, umbilical region, and urinary
  • Obstruction in the passage of
  • Decreased

Line of treatment of Anal fissure:

The anal fissure can be managed or prevented by correcting constipation to a large extent. It can be corrected by:

  1. Drinking an adequate amount of fluids (softens the stool and makes the passage easier).
  2. Intake of food items rich in fibre such as vegetables, fruits, and brown )
  3. Intake of bulk-forming agents such as psyllium bask and
  4. Avoid the usage of excess spicy food items, red meat, and fast
  5. Avoid excessive straining while passing
  6. Intake of milk and buttermilk in the diet.

The treatment plan for anal fissures can be oushada seva (non-invasive treatment) or surgical plan.

  • Medicinal supplements include various preparations of Guggulu or can be selected according to the dosha
  • Non-invasive therapeutic procedures include:
  1. Piccha Basti with Madhu, yasti, and Krishna tila can be
  2. Anuvasana basti with Yashtimadhu
  3. Sitz bath/ ushna avagaha sweda- sitz bath must be performed with warm Triphala powder can be added for wound healing.
  4. Application of herbal oils like jatyadi and nirgundi oil helps in the healing of the wound and eases the passage of the wound and eases the passage of stools.
